* Invoices will be issued that meet the minimum requirements of the Australian Taxation Office and applicable legislation and indicate the GST component.
** Charges to be raised at the time of issue of these items. Credits will not be allowed if any of these items are returned unless the items are faulty or unsuitable for the purpose intended, as they are purchased outright by the Centre.
*** Reminders are sent for non-return of these items allowing for return within 28 days. Credit of 75 % of the per item cost will be given for items returned within 6 months of the invoice date.

Other Services Provided
Urgent Special Issues : $161.70 per issue
- The issue of monitors is normally performed on the first working day of the week. There will be no charge for the special issue of extra monitors which can be done on this day. However, when monitors are required urgently and the issue is not done on the normal issuing day, then the above special fee will be incurred.
Special Handling Fees : $161.70 per year
- Where special arrangements for the issue of monitors or Reports have been requested by the Centre (eg special packaging or labelling) the above special fee will apply.
E-mail Services : FREE
- An electronic copy of the Dose Report can be sent in addition to the normal hard copy. Only one e-mail address can be nominated per Centre. Where more than one Centre share the same e-mail address, reports are concatenated and sent as a single file.

Dose History
- The Dose History of a worker from the time that monitors issued by the Laboratory were first worn can be retrieved from our records. Prior to 1987, all records for the Personal Radiation Monitoring Service were hard copies that have been archived. Since 1987, these records have been stored in a computer database. The charges for compiling a Dose History for a wearer is as follows:
