Accident Notification Form

The Regulatory Guide: Reporting an Accident (PDF 52kb) (updated July 2010) is provided to assist licence holders in their compliance with regulation 46. The document defines an 'accident' for the purposes of regulation 46 and provides information on when and how such accidents must be reported.

Accidents should be reported using the Accident Notication Form (Doc 57kb) (updated July 2010).

Disposal Request Proforma (Regulation 53(1))

Disposal Request Proforma (Regulation 53(1))(DOC 89kb) (updated July 2010)

This form is to be used to seek approval from the CEO of ARPANSA to destroy or otherwise dispose of controlled apparatus or controlled material. Disposal includes the transfer of controlled apparatus or controlled material to a person or organisation outside Commonwealth jurisdiction. Please refer to the Regulatory Guide for further information.

Nuclear Installation Licence Application

Applicants who wish to apply for a licence for a nuclear installation should refer to the  REGULATORY GUIDE: Applying for a Facility Licence for a Nuclear Installation for guidance on how to complete a  Nuclear Installation Facility Licence Application Form.

Prescribed Radiation Facility Licence Application

Applicants who wish to apply for a facility licence for a prescribed radiation facility should refer to the  REGULATORY GUIDE: Applying for a facility licence for a Prescribed Radiation Facility for guidance on how to complete the  Prescribed Radiation Facility Licence Application Form.

Quarterly Reporting Proforma

This proforma has been developed to assist licence holders in fulfilling their quarterly reporting commitments to the CEO of ARPANSA. It is designed for use by holders of a source or prescribed radiation facility licence.

Regulation 51 Request for Approval

This form is to be used to seek approval from the CEO of ARPANSA to make a relevant change that will have significant implications for safety under regulation 51.

Source Inventory Book

The Source Inventory Workbook (SIW) is the form approved by the CEO for licence applicants/holders to record details of all controlled material and controlled apparatus to be dealt with under an ARPANSA licence. An explanation of terms and required information is provided on the first worksheet.

Source Licence Application Form

Transfer Notice Proforma (Regulation 53(2))

This form is to be used to advise the CEO about the transfer of controlled apparatus or controlled material to another organisation within the Commonwealth. This form must be received within 7 days of the transfer. Transfer of sources outside Commonwealth jurisdiction is deemed disposal and requires the prior approval of the CEO.
