PRMS Forms
To use the Personal Radiation Monitoring Service (PRMS), you must first register with us by completing a Centre and Wearer Registration Form.
Registration Forms
You must also provide the name and birth date of each wearer so that a unique wearer identification number can be allocated. The wearer identification is used in the accumulation of a dose history for each person. (If monitoring is required for extra people at a later date, this can be arranged on request.)
